This is a quick job for experienced users. I have a psd file (pages and images set on each layer) you will just convert psd to html or xhtml/css

but I need you to create a working contact us page. It should include
NAME
EMAIL
PHONE NUMBER (optional)
MESSAGE
-CONFIRMATION OF MESSAGE SENT.

we will receive messages in an e-mail.
