Hi,

I have a new website and i have a new form started. Only the box are in place. I need someone who can code the form for the 3 selection + text box go to my email when someone press submit. (See printscreen atached.)

Programmer will need to add few details i will provide like service selected and city the people will choose before adding the question in the text box… i also want to be able to modify the form in the future to add new services or country…

IMPORTANT: form are already in place in my design with the button and all. You need to code the form but no modification to the form look. This need to be only back end coding no front modification.

In your PM please specify what you will do and how i will be able to add categories of services or selected country in the future. This need to be a perfect job verry fast!
