Freelance Project

I need one form to be created for website and print. It should be delivered in the following format

1. HTML & CSS
2. In word Document
3. In PDF

The form will contain radio buttons, list down menu, text field, check buttons… etc. It should be one that users are able to input intuitively and contain a functional submit button for users to submit via email.

I’ll provide the fields to be included in the form before I select your bid.
