Survey Software

I am in need of an individual to develop a php site that does the following:

1) The sites goal is to have users be able to create surveys with this system and have the information inserted into a MySql database.

2) Upon creating a Survey, the survey needs to be able to be taken by a user with there select answers and those pieces of information also needs to be stored in the MySql database.

3) I need the results per survey to be measured and tallied in another page, where simple line graphs or bar charts can display the results ( I.E. a bar showing 35 out 55 people taking this survey like chocolate icecream)

4) When being able to create surveys, the user must be able to select from different types, Radio buttons, drop down, check boxes. etc

5) This needs to be a similar system to www*surveymonkey*com.

This should be able to go live at completion and be on the SAME or Better than the professional level of surveymonkey*com
